How do I become an Approved Attorney?

1. Complete the Approved Attorney Application (click here to download PDF version or click here to download MS Word version). Make sure to save a copy of the pdf form on your computer.
   
   
2. Send the completed application along with a copy of the Declarations Page of your Professional Liability Insurance Policy by email or fax to your local office.
